Some of the documents we use in our practice are reproduced here in several common file formats. You may download them to your computer or view the PDF version online. PROFESSIONAL SERVICES AGREEMENT
This is a sample of the written agreement we use to describe our relationship with you and to identify the obligations we have to one another. You will notice that it includes several alternate paragraphs for the payment of our fees and expenses. These are designed to meet the differing needs and preferences of our clients, which may vary from one case or client to another. You may wish to examine the terms of this agreement before our first meeting so that we can answer your questions at that time. Once you have decided to retain our services, we will prepare a final version of the agreement for your signature and ours which is unique to your case.
CLIENT INFORMATION FORM
In order to represent you effectively, we will need to know a little about you and about your past criminal, medical, psychiatric, educational, employment, and family history. You may use the form provided here to furnish us with this information. Download it to your computer in one or more of the file formats shown. You may then complete the form by using your computer keyboard or by printing it and filling in the information by hand. Don=t worry too much about the completeness of the information. Just fill in as much as you can remember or can easily look up. We will go over it with you at a later time.
GENERAL RELEASE FORM
It is sometimes necessary for us to obtain records of your past and to discuss confidential aspects of your life with attorneys, doctors, teachers, and others who have provided you with professional services. In many cases, such information cannot be disclosed to us without your consent. The general release form provided here (Authorization for Release of Records and Information) is usually accepted as evidence of your permission to reveal and discuss such information with us. We recommend that you read this form carefully, download it, and complete it on your computer by filling in your name, birth date, and social security number in the spaces provided. You may then print the document, sign it, and bring it with you to your first meeting with us. If you are unable to complete the form on your computer, or if you are uncertain about how to complete it, we will help you with it at a later time.
